(1) (a) An applicant, licensee, or certificate holder shall send the division a signed statement, in a form required by the division, notifying the division within 10 business days of a change in mailing address. (b) When providing a mailing address, the individual may provide a post office box or other mail drop location. (c) In addition to providing a mailing address, an applicant, licensee, or certificate holder may provide to the division, in a form required by the division, an email address and may designate email as the preferred method of receiving notifications from the division.

(2) An applicant, licensee, or certificate holder is considered to have received a notification that has been sent to the most recent: (a) mailing address provided to the division by the applicant, licensee, or certificate holder; or (b) email address furnished to the division by the applicant, licensee, or certificate holder, if email has been designated by the applicant, licensee, or certificate holder as the preferred method of receiving notifications from the division.
