(1) In accordance with Title 63G, Chapter 3, Utah Administrative Rulemaking Act, the department shall make rules regarding: (a) which vaccines are required as a condition of attending school; (b) the manner and frequency of the vaccinations; and (c) the vaccination exemption form described in Section 53G-9-304.

(2) The department shall ensure that the rules described in Subsection (1): (a) conform to recognized standard medical practices; and (b) require schools to report to the department statistical information and names of students who are not in compliance with Section 53G-9-302.
