§ 42-14.6-9. State patient-centered medical home program expansion. (a) The director of the department of administration is hereby authorized to expand the current patient-centered medical home program for state employees and retirees with chronic health conditions that are covered by the state employees health benefit program and are high frequency healthcare utilizers. This program shall be in addition to and shall not alter the Rhode Island All-Payer Patient-Centered Medical Home Act as set forth in § 42-14.6-4.
(b) For the purposes of this program, "high utilizers" means individuals who are among the top one to five percent (1-5%) of utilization within their payer group.
(c) "Patient-centered medical home" means a practice that satisfies the characteristics described in § 42-14.6-2.
