640.060 Duty of Commonwealth's attorney. It shall be the duty of the Commonwealth's attorney after the imposition of youthful offender sentence, unless the defendant is granted probation, to cause to be forthwith transmitted to the facility or program with the commitment papers, a concise statement of the facts adduced at the trial or at the hearing of a plea of guilty.
